Maybe because I never use those urinals?
 
I don't know any language but English, but I am guessing: The logical choice for the second person would be either the third or fifth urinal so no one would be right next to another, but the second person chose the fourth one which now makes it impossible for another person to not be directly beside someone. It is more or less an unofficial rule that you take every other urinal unless it is so crowded that most urinals need to be used.

Is the joke that you're not supposed to 'check' the penis of your 'mate'? Or does that not work in French?

Yes, I suppose it's rather about the position... given the "échec" expression.

Still, you simply just not look. Why should you? You do not even has to look to the other guy's face.
 
What confused me is the static position of the guy away from a urinal... had he not been so close to the other one, but one the other end of the row, I would have assumed it was a dynamic position, entering the room (if one is concerned about "peepers", why stand right next to the entry door?). I took it he was standing and looking, even the idea that he got flustered by a third guy somehow spoiling whatever game he might had by standing next to the peeing guy crossed my mind.
