The penis and scrotum have sebaceous glands. Occasionally, it is possible to get an occluded pore that fills with oil (called sebum) that leads to an infection. These infected pores are called comedones. It has nothing to do with being clean or dirty- any more than acne on the face- the bacteria that cause them occur naturally on the skin.

Use a topical cortisone creme to take down some of the inflammation and tenderness. If it is a comedone, it will typically form a head and drain on its own. If it does not drain and it continues to be painful, gets red and warm to the touch or it forms a cyst, you will need to see a dermatologist.

Usually these resolve on their own without any additional treatment.

Please don't go to a doctor for a ZIT.

Ya I've had that a few times. Once I went to the doctor cause I was worried, but he literally said it's just a zit. I sorta felt dumb (although I went for an overall STD test so it wasn't a wasted visit). I do shave/trim around there as well, so he said that the zit could have developed from an ingrown hair. Like yours, it was painful and had a white head. I eventually popped it.

A friend of mine recently told me he had something called a skin tag in his underarm. I had no idea what a skin tag was. So I looked up some information on line and see that these skin tag things are also common on the genitals. While more common on older people they occur where skin rubs fabric or there is friction.
They look like a zit some times.
I just bring it up because I had never heard of these and would thing of them maybe as a zit or something had I not learned about them.
