Hi there Kara_Bulut & others...

Just wanted to get some advice on this.

For like a year now (though it's come and gone and I figured it's normal so I let it keep happening) I've noticed that I have what feels like a little nick at the front of my anus.

I've never noticed it be worse after anal sex (which I haven't been having regularly anyway) but it does cause a bright red spot of blood on the toilet paper after most bowel movements and it makes wiping my ass a bit stingy and uncomfortable.

I've looked online about fissures and it says that they only happen at the front about 2% of the time or something like that, and usually only occur at the back of the anus.

Anyways, what would you recommend I do? I can see a doctor and will, but in the meantime. Are there suppositories I can get over-the-counter to help the problem? I don't want it to become more chronic than it already is.

Also, it feels like maybe anal sex has stretched the skin permanently around the front of my anus. I've had two boyfriends that were pretty big admittedly, and I haven't always taken my time working up to it in sex... I often move too fast and don't give myself enough time to warm up. Is it possible I've caused some sort of defect in the skin at the front of my anus? It feels like there's a bit of a fold in the skin (like the skin forms a V-shape where my taint/prostate area between my legs gives way to where the anus is) and that's where it always stings.

Also, I know it's more of a nick and not something else like an STD, as I have had anal warts in the past and it's very specific to the front, it's not like when I had warts.

I'd recommend warm baths with epsom salts in the mean-time. Avoid rough toilet paper. Perhaps use some wipes instead. Increase your fibre intake and avoid straining whilst having a bowel movement.

And see a physician!

As for stretching it out, I wouldn't worry too much about it. It's almost a non-issue.

Update

This is frustrating...

Went to the STI clinic today... the doctor couldn't find anything, not even a fissure. He just said that I was feeling a small bump/fold of skin at the front of my anus and that I need to use more lube from now on and relax more when bottoming in sex.

It feels like there's a crack of skin there, and a bit of blood gets on the toilet paper. Yet he said he couldn't even see a fissure. He described what he saw as a little fold of skin, that there wasn't a fissure but rather a tiny "pinhole" at the front of my anus.

Is that typical? Is it an improperly healed tear?

What can I apply to it? Are there non-prescription ointments that would help heal it better, since it's always irritated?

The incidence of anterior tears in adult men is extremely low. If there wasn't a visble tear visible in the exam, then it's unlikely that you have an anterior tear.

I'm going to recommend two things to you-

The first suggestion is that you should see a gastroenterologist- a doctor who specializes in disorders of the stomach and intestine.

The second suggestion is that you should just relay your symptoms to the physician and not try to diagnose this yourself. The problem that can happen in these situations is that when you go in saying, "I think I may have an anal fissure" and the doctor will do an exam and say, "No, you don't have a fissure" and that's the end of the examination. If you go in saying, "It doesn't feel right, I'm having some pain and I see bleeding", then you're more likely to get a complete examination and the physician is more likely to look for all possibilities.

I won't say it to the doctor, but it seems to me that it's a "sentinel pile".

That's who I see for anal issues.

The specialty names vary between countries but "gastroenterologist" is a broad term for doctors who specialize in treating disorders of the digestive system.

A proctologist (or more accurately, a colorectal specialist) is usually a surgeon who specializes in surgery on the colon, rectum and anus.

It makes more sense to see a gastroenterologist because you don't have a diagnosis.

You've seen a doctor. It sounds like he examined your anus and didn't see any signs or symptoms of a fissure. So, now you're really looking for someone to determine where the blood and discomfort is coming from. That's going to involve additional tests beyond just a visual and digital exam and that's not something a family medicine doctor would not be able to do- it's more of a specialty.


And if this last doctor couldn't see what he considered to be even a crack, then would it at least do no harm to buy an ointment to help soothe the area?

You can always buy an over-the-counter preparation like Tucks pads or a hemmorhoidal ointment with hydrocortisone. Both are generally soothing and help irritated anal tissue.
 
I hope it's not serious :( I have an appointment made for Friday... hopefully will get a referral. Should I ASK for a referral or just see if the doctor gives me one?

The only thing I can imagine is it's a sentinel pile, from what I've read. The skin at the front of my anus feels stretched, and like it's a tiny bit torn or something. Not sure how to explain it.
 
How common is a hemorrhoid at the front of the anus?

Hemorrhoids can occur anywhere on the anus- internally or externally.
 
I have been diagnosed with a small fissure to the REAR of the anus (apparently the most common place for them). The annoying thing about it is that it tends to create a bacterial infection around the anus where there is a lot of wetness/pus-like fluid, tremendous itching (usually at night) and painful bowel movements with blood on the toilet paper. The severity of it comes and goes (and ALWAYS improves when I am on antibiotics), but I have never been able to get rid of it completely. If anyone has any advice for getting rid of this once and for all, I would be eternally grateful! Especially, since I would like to wear white/light colored underwear and pants again, seeing as it is summer and everything...

And they're sure that you don't have a pilonidal cyst?

I went to several doctors, including one of Norway's leading gastroenterologists who had an extensive look around and in the anus, and he was very certain that it was due to a fissure. He was talking about how I had an unusually "tight" ring and that surgery to open up the right was always an option, but he didn't recommend it for me. He also said to use detergents without perfume, wash the anus rather than wipe with toilet paper (not an option for work) and smear with peanut oil... All those things do help to a certain degree, but it always comes back again. Ciprofloxacin works very well, but I'm usually off of that again in 10 days...
 
Alright, well I've been applying Anusol and literally after one day of using it, I've completely healed up. No puffiness or irritation, just back to the way I was normally. :)
 
Okay so the problem had come back intermittently and I noticed the skin at the front of my anus felt like it had lost elasticity and was always irritated.

Just to conclude this thread, what happened is I had my doctor look at it and she said that there certainly was irritation, and that it looks like there had once been a tiny tear (this would have been sometime a year to two years ago... I had chronic diarrhea all the time back then and also my boyfriend was really well endowed :P) and that it looks like the skin was irritated still and had thickened a bit. (This would explain the loss in elasticity I felt).

I've been prescribed an ointment with a steroid in it which should fix me up and if not then I'll be referred to a gastroenterologist.

So... I'm happy I finally got it looked at because it should be very easy to fix! :)
