It's never really hurt, but I can't pull my foreskin back that much (that picture is how much I can before it hurts).

You should consult a doctor and ask that you be considered for a "dorsal slit"....its NOT exactly a circumcision...its just a small incision on the dorsal side of your penis that will allow retraction of your foreskin...take like 15 mins to do...and like 3 stiches...:wave:
 
That looks painful if you roll back fully.

You have to work (roll back & forth) your foreskin so often until it become loose.
 
Based upon the pictures, you have two issues-
  1. You have phimosis (inability to retract the foreskin) along with a tight foramen (the hole in the foreskin that the head of the penis should fit through)
  2. It also looks like you may have a fibrous ring a little further into your foreskin that is tight. This is what appears to be causing the issue.
There are several non-surgical options that you can talk with you doctor about. These options involve trying to stretch and loosen the foreskin over time. However, if you do have a fibrous band, you will probably be unable to stretch the foreskin because in most cases the fibrous band will not stretch.

The first surgical option is a preputioplasty or a dorsal slit that will cut through the fibrous area and may also add a small slit on the edge of the foreskin to allow it to expand.

The last surgical option would be a partial or complete circumcision.

You'll have to see your doctor to find out what your options are. You may be referred to a urologist for a second opinion.
 
You need to ask yourself does it hurt, does it interfere with sex/jacking off or cause other physical problems?

If the answer is yes to any, then seek proper medical advise not just from your general doc but from a urologist with experience in this matter. If your concern is mainly about cosmetic look of your head not fully showing then do not bother with any medical procedure.

We occasionally see guys in the emergency department with paraphimosis. They're shipped immediately to the operating room.
