So recently, while building up to ejaculation, I have started to get these unbearable headaches. It starts almost immediately after I start stroking, and it gets more and more intense as I go on. I have to stop right before I orgasm, it gets so bad. It relatively quickly dissipates.

If I beat through it, (lol), and orgasm, it gets ten times worse. And it stays for forty minutes or longer. I haven't changed anything in my diet, anything in my sex life, anything at all.

What is going on?

It sounds like coital cephalgia (CC) or extertional migraine (EM).

These type of headaches are connected to physical exertion, including sexual activity. Most of the time, the headache is connected with orgasm and ejaculation which means that your pattern is slightly atypical subacute pattern since your headache starts well before orgasm.

Other typical features:
  • Typically, the headache affects the back of the head and the neck and may include facial flushing.
  • The acute phase of the headache lasts a few minutes to an hour but the residual portion of the headache may go on for a day or more afterward.
  • The headaches may happen suddenly and occur for several weeks or months anytime the person has sex and then they may remit with no headaches for months or years when the person has sex.

Since this is something new, you should see your doctor. There are things that need to be eliminated as possible causes before a diagnosis of coital cephalgia is settled upon. And if it is CC/EM, then the doctor may want to try preventative and rescue migraine medications. Regular exercise, dietary modifications, accupuncture and biofeedback may also help.

Well, it isn't physical exaustion. I just played some football with my friends, and was fine. So this is only sex.

This is really bothering me. Thanks for you help, Karabulut.

Not exhaustion- exertion.

These headaches are a type of migraine that is associated with rapid breathing, increased heartrate that comes with physical exercise. Researchers believe that coital cephalgia is a type of exertional headache- it just happens to be triggered by sexual activity and ejaculation instead exercise exertion.

These migraine syndromes are what we call a "diagnosis of exclusion". That means that testing is done to eliminate other causes. Once the other possibilities are eliminated, the last possibility is migraine. So, please do see your doctor about your symptoms so that you can get a proper diagnosis.

Check with a doctor... don't worry about telling him exactly everything you just shared here.
See what he says.
I'd like to know if this would happen if you were having sex with a partner if the same symptoms would occur.

Hey I had the same problem a few years back while I was in Thailand....I thought I got something there... EXACT same symptoms... I went to specialists had scans and when tests where done a week later, my symptoms stopped and he said it was something that happens (I forget medical term he used) to guys in 20's or 30's rarely and stops as quick as it starts (1-2 weeks), and they don't know what causes its and usually nothing bad.

You lean forward when you're jerking off don't you? It's only an overflow of blood to your brain. Stop leaning forward and you won't get those headaches again.

And perhaps you should have a blood pressure check, just in case.

if they dont go away maybe you should see a chiropractor...
alot of times when headaches have no cause that doctors can find your neck may be just slighty off balance, thats all it takes to put pressure ona nerve and BAM give you a headache

The same thing happens to me, I discovered it happens when I haven't had enough water that day. Try drinking a few glasses and see if its better.
