strange results from std test

I got an std test performed, I didn't think I had any std's but I had never gotten one and really just wanted to know I was HIV-. Anyways, I had to give blood and give urine for the test.

My doctor called me a few days later and said I was std free, but she was concerned because there were red blood cells in my urine, and she wanted me to come and take another urine test to make sure it wasn't some sort of fluke or something. I went back about a week and a half later, and both times for the test I peed into a cup up to a certain line, then the rest into a second cup. Apparently, both times I took the test, they found red blood cells in the first cup, and none in the second. and both times there were no white blood cells in either cup. She said she didn't understand the results, and her colleagues didn't either. She had me come in for a genital exam (which was kinda scary) and everything looked healthy. She gave me an antibiotic to take (the stuff they give people with clamydia) and said I should come back in 3 weeks for another urine test and we'll take it from there.

I haven't had any pain urinating, and I can't see any blood when I pee. now i'm freaked a bit every time i go to the bathroom! Does this seem weird at all? any opinions? thanks ;)
 
I have a condition called "chronic prostritis" (not sure of the spelling) and have the same symptoms. My uroligist said that basically it is an infection of the prostrate and antibiotics clears it up. I get blood in my urine when I first start to pee and that seems to flush it out. I have had this like four times spread out over the past thirty years. Once,twice in a year, and then spread out over time. My doc confirmed it by doing a test with a scope in which they knock you out and then pass a scope up the urethra to check out the prostrate. The only other follow up is a prostrate exam and psa test once a year. Sorry for the spelling mistakes.
 
Something that is a little confusing-

A urinalysis checks for two types of blood cells in the urine- white and red.

White cells (pyuria) in the urine is often a sign of a urinary infection and antibiotics would often be prescribed. Some drugs like Tylenol (acetaminophen/paracetamol) can cause a false positive, however- so it's not always a reliable test.

Red cells (hematuria) in the urine can be a random/temporary finding or it can be a sign of trauma or kidney disease. The hematuria can be there even though the urine is not red to the naked eye (which is called occult or microscopic hematuria). If a repeat test is negative for red cells, there would not usually be any treatment warranted- just a followup test. If the repeat test is positive for red cells, then the doctor would look for an underlying cause like a kidney stone.

From the description, it sounds more like your doctor is treating you for white cells in the urine.

In either case, if you have a positive urine test followed by a negative test, it's usually not anything to worry about if you don't have any other symptoms. In 3-6 months, have a repeat urinalysis done to ensure that there is no problem.
 
Thanks for the feedback! The prostiris seems like it is something I should look into, although when I had the genital exam the doctor did check my prostate and didn't find any problem with it. I'll mention it to her when I go back for the third urine test. Hopefully I don't have anything to worry about :)
 
Karabulet, I think he had red cells in both cases. But when he went back they had him pee in two cups... one at the beginning and then one part way through.

Basically, what you are describing sounds like the red cells are coming from somewhere distal to the bladder (in the urethra). That's why there are cells in the first cup but not later (b/c you've flushed them out). Actually, I'm kind of impressed that they did it this way. I'm not sure what the antibiotics are for except they are probably attributing it to urethritis (the most likely reason). However, if you use sounding rods or anything that damages the urethra slightly, it could be the cause.
 
Sorry for the typo KaraBulut. And just to clarify, both times I went in for a urine test they had me pee into two different cups. It was the same test both times. And I don't know what sounding rods are, but if its something you stick down your pee hole, I havent been doing that. I don't really know why they gave me antibiotics either, except that they wanted to give me some sort of treatment just in case there was an infection, or possibility of infection.
 
There are a number of STIs that are not able to be tested for on routine tests. Mycoplasma genitalium is one. I think only two labs in the world can test for that. And that can cause the red cells in the first cup.

Good news though. Standard antibiotics used for STIs kills mycoplasma genitalium.

"Non-specific urethritis" used to be a common diagnosis for such symptoms/infections. Then we thought that chlamydia was responsible for most of this. Now we're beginning to recognise there are yet more undiscovered STIs.
 
Now i agree something sounds a little off with this analysis -
1.) no white cells in ur pee
2.) red cells on both occations
3.) standard sti check normal ( chlamydia,gonnorhoea . . .)
4.) no symptoms

now i would entertain two things personally
1.) some other cause of urethritis such as mycoplasma or ureaplasma etc . . .
however my personal favourite would be THIS
2.) asymptomatic haematuria which is a LOT more common than the atypical organisms for urethritis
