Oh Shit. This isn't good. You were wondering what direction this virus would take?

About a month ago, there was a discussion in this thread about mutation and how viruses tend to mutate toward increased infectiousness and lower mortality. There was also a discussion about how SARS-CoV-2 has a spike protein on its surface that it uses to infect human cells and the virus' dependence on that spike to infect humans limits its ability to mutate because a mutation that affects the spike also affects the infectiousness.

That is what is happening- the virus has mutated on its spike in a way to make it more contagious. The studies are saying that the new mutated virus (G614) is more contagious but the new strain is not less lethal; it can still make people very sick.

The vaccines being developed were based upon the older strain (D614) of the virus. Now that it has mutated and the US has managed to allow nearly 3 million people to be infected, the vaccine researchers are going to have to ensure that their vaccine works on the new strain (G614).

A global study has found strong evidence that a new form of the coronavirus has spread from Europe to the US. The new mutation makes the virus more likely to infect people but does not seem to make them any sicker than earlier variations of the virus, an international team of researchers reported Thursday...

The study, published in the journal Cell, confirms earlier work suggesting the mutation had made the new variant of virus more common. The researchers call the new mutation G614, and they show that it has almost completely replaced the first version to spread in Europe and the US, one called D614...

"Our global tracking data show that the G614 variant in Spike has spread faster than D614," theoretical biologist Bette Korber of Los Alamos National Laboratory and colleagues wrote in their report. "We interpret this to mean that the virus is likely to be more infectious," they added. "Interestingly, we did not find evidence of G614 impact on disease severity."

This could be good news, said Lawrence Young, a professor of medical oncology at the UK's University of Warwick, who was not involved in the study.

"The current work suggests that while the G614 variant may be more infectious, it is not more pathogenic. There is a hope that as SARS-CoV-2 infection spreads, the virus might become less pathogenic," he said in a statement.
 
On the vaccine front, the news is good. Several vaccines being tested on humans have triggered an immune response and some of the vaccines triggered the test subjects to produce antibodies in equal or greater amounts than people who contracted COVID-19.

In Congressional testimony this week, Senators pressed the HHS and FDA for answers. The officials were evasive about the government's involvement to help fund the vaccine efforts- saying only that the US was involved in "more than one" of the vaccine development efforts. The official from the HHS did say that he felt that a vaccine would be available by the end of the year... although other experts like Fauci have not been that optimistic when asked about the vaccine prior to the Congressional hearing.

One important thing that is mentioned in the article below: the WHO is coordinating and collecting data on over 100 vaccine trials going on across the globe. The person sitting in the Oval Office has announced that he is withdrawing the US from the WHO and that will affect the US' ability to be an active participant in vaccine development. If a country in the WHO, like China, develops a successful vaccine, our lack of participation in the WHO research efforts may also affect our ability to get a vaccine for our citizens.

In the United States, some questions remain around the nation's Operation Warp Speed program -- and no specific answers were given in a US Senate Appropriations hearing on Thursday about which companies and how many Covid-19 vaccine candidates are in or being considered for the program.

When Wisconsin Sen. Tammy Baldwin asked for details about the vaccine prospects, Gary Disbrow, acting director of the US Biomedical Advanced Research and Development Authority, said he couldn't divulge that information yet -- but hopes to announce the entire portfolio soon.

This is how bad I feared it would get back in the early days. I thought that North America would learn from Spain and Italy and do everything possible to avoid this unnecessary crisis in the health care system.

That they are using ECMO before mechanical ventilation was a big surprise to me, too. Once upon a time, ECMO was only used in neonates and was rarely used in adults.

It's been an interesting finding. Because of the loss of compliance and the fibrosis in ARDS, it requires increasing pressures from the ventilator to oxygenate ARDS patients. The high pressures cause barotrauma which damages the aveoli and can cause pneumothorax. There were questions whether the disease process was killing ARDS patients or whether it was the treatments that killed them. If ECMO does increase survivability compared to mechanical ventilation, that may change the way ARDS is treated in the future.

All of this has to be a nightmare for the nursing staff. Typical staffing in the ICU is 1 nurse for every 2 patients. When you start adding ECMO and other advanced treatment, the ideal staffing can become 2 nurses to 1 patient. In NYC, there was speculation whether the shortage of ICU and ED nurses may have increased the mortality of COVID-19 patients in March and April. It reached the point that the hospitals in NYC were using medical students and new graduate nurses who did not have their licenses yet.

Almost all the interviews and articles I've seen about her seem to originate from her admirers. There is so little negative criticism it makes me think her public image is very tightly controlled. She disables the comments on her videos, she says it's because she often gets death threats.

Is she as well known in the USA as her own publicity suggests? Is she taken seriously?
 
Meanwhile, down under, the state of Victoria in Australia is seeing a spike. From near elimination a couple of weeks ago, to over 100 new cases yesterday. Apparently security guards put in charge of quarantining new arrivals from overseas were lax and easily bribed and allowed those in isolation to mingle, and the guards took the virus back to their communities.
