It has more to do with flexibility than size. I've seen guys with 9's who can barely touch the tip, and I've seen guys with 5's who can take it down to the balls.
 
I thought it was physically impossible to get it up if it was over something like 15 due to the amount of blood needed from elsewhere makes you faint.
 
You know what, that one is probably fake.

And any real one that size would likely not be the result of above average, but still healthy development. It would probably be due to some kind of medical condition that the person would be seeing a doctor to try to stop, and which might leave the guy with other side effects.
