I think the advice from Nutsac, Josher and Riverrick can't be faulted.

I myself go for leather cockrings/straps as I'm shit scared that once I got my cock and balls through a metal one, I'd stay too thick and stiff for too long and not be able to get out of it without an embarassing visit to an emergency department of a hospital or the local hardware store!!

I think it was painful because I got super-aroused, and freaked that it wouldn't come off, etc. The second time I used it, It wasn't as bad. I just won't use it daily.
