So, unless the bottom and top are gifted (or are getting a little help from the pharmaceutical industry), there's a lot of going soft, getting hard and going soft again. There's usually porn (often straight porn) going in the background and when there's a close-up, the guys go off set, jack off to straight porn, return to the set and then the close-up is done.
