My symptoms are as follows:
- "scraping" sensation every bowel movement I have - this feeling is toward the front of my rectum
- feeling of something in my rectum like when one is constipated
- "bearing down" or loose sensation back there
- other times, feeling of pressure in rectum
- feeling of incomplete bowel movement / needing to go even after I'm done
- "bearing down" sensation remains after a bowel movement
- no IBS/digestion problems for the past 6/7 years - good digestion
- feeling of blocked defecation or that bearing down doesn't push anything out - feels as though I am pushing it out and then everything just goes back up
Other facts about me:
- 28yo male, healthy weight, active
- used to bottom; haven't in 6 years since my symptoms started but my last two bf's were pretty well endowed and I worry that this might have caused injury
- I had anal warts ~6 years ago, including internally, which were treated with cryotherapy/liquid nitrogen. I wonder if it's possible these caused scarring and that contributes or is a cause for the irritation?
- sigmoidoscopy 6 years ago showed rectal papillae but was considered normal
- drinker of coffee and diet pepsi but trying to cut back a lot lately
I am concerned it could be one of two things, based on symptoms, and because the issue has not gone away after 6 years of symptoms:
- rectal (internal/mucosal) prolapse when I have a bowel movement
- dyssynergic defecation (although that doesn't account for the 'scraping' sensation internally)
KaraBulut and others might remember me posting a few years back about what was initially anterior anal irritation (a poorly healed fissure or something) and then other related concerns. I have booked an appointment to finally face this and see my family doctor in the new year for the soonest available appointment. It's been a few years now, and sometimes I just accept it and get on with things but lately I've been having the issues more noticeably again and I can't handle it; it makes things very uncomfortable for me. Last time I mentioned it to my doctor, she suggested referring me for another sigmoidoscopy. I don't see the point because last time I had one, a few years ago, nothing abnormal was discovered and I don't feel as though anything has changed since then.
Questions:
Is this the feeling I described above what someone would feel if they have an internal prolapse? The scraping sensation, and the feeling of pushing something out but then it moves back up? (instead of defecating successfully where you feel it coming out and then it leaves the body). This feels like it is happening towards the front of my rectum (and the front of my anus is where I used to have so much irritation). I apologize for being graphic but it feels as though my rectum is doing the movement of pushing something out, and has traction. But then it goes back up inside. A part of me thinks it's just the feeling of bearing down and the muscles squeezing but I can't tell.
Could this same symptom just be dyssynergic defecation because I hold so much tension down there? I am completely open to the idea that the anal irritation I felt a few years ago has made me very uptight and resistant to bowel movements because I associate it with discomfort. I notice that I can be very tense and feel like maybe my pelvic muscles are tense and not properly coordinated when I'm having a bowel movement. But I also don't see how this would cause the symptoms I have when I'm not going to the bathroom and it also doesn't account for the scraping feeling I get when I do a #2.
Is there any point in asking my doctor for a defecography referral? I feel like maybe I have some type of prolapse condition not detected on a sigmoidoscopy because it only happens when I have a bowel movement and bear down? Or is there any other way I can get them to take a look at what is going on?
I feel like because of my young age and because the symptoms sound a lot of IBS type stuff, they don't take my concerns seriously. But I also feel like my concerns are related to when I have a bowel movement or bear down, and so it goes undetected. And, especially, it's a difficult/awkward thing to bring up in detail with a specialist or doctor who basically gives you time to say two sentences before they send you on your way.
