Now that you mention it, there does seem to be a more (painful) sensation when lying on my back...but I think the eroticism of the kissing, holding, sucking, etc. that happens in that position distracts from it.

I never felt used but I definitely felt guilt. I was raised in a Catholic family, going to parochial school (taught by nuns, later by brothers and priests). We had religion classes ever day. While sex was never explicitly mentioned (until briefly in high school health classes in the all male Catholic school attended) being "chaste and pure" was but that was mainly aimed at girls. I felt that guilt for many years.

I know exactly what you mean - even though I was not raised very religious we felt guilty as hell. After all - fucking was something "homos" did and we were not gay just horny!
