So I went to a Urologist that my MD recommended to see about my phimosis - the Urologist walked in, very nice guy, and before I could explain my situation in detail his answer was "The only way to treat this is circumcision" - I told him that I understood after researching on my own that circumcision was an option and would consider it as a last option. I talked about the steroid cream that i've seen many people on the internet and even Doctors in Medical Journals recommend as well - my Urologist waved it off as a joke and that he the steroid cream as the equivalent of a placebo pill. I then began telling him of my concerns being circumcised and was honest that I do enjoy having a foreskin and really don't want to just cut if off.

He ended our session with saying "there are two scenarios. One - you keep the foreskin and run the risk of having pain when you have intercourse without a condom and run the risk of having health problems when you get into your late 50s. Two - you just get a circumcision and you don't have anything more to worry about."

He then gave me his card, asked me to e-mail him, and most importantly let me know when i'm ready to have the surgery.

Sigh - I am not questioning his medical knowledge since he does have a really good reputation but I just felt as though I was being pressured into going with a circumcision without discussing other alternative methods out there. It's not like I can't pull back the foreskin at all - it's just that I can't pull it back when it's hard.

Now i'm freaking out wondering whether he is right or whether I should look into seeing another Urologist, one that would be open to discussing alternative methods.

At the moment I'm in school to become a Nurse and while there are vast distances between nursing and M.D. training; patient interaction is *not* one of them.

Get a second opinion, your doctor is just giving you his blanket response to the issue

think of it like how back in the 70's if anyone had a sore throat, out with their tonsils! Whether they needed them out or not

^ correct. there are some cases where the cream does not work. but in these cases often a partial circumcision can work. But to tell you that even the best doctor has to have a look at your penis.
If without looking at the situations he tells you right from the start that you need to be cut - he only wants to make money.

Unfortunately, surgeons aren't known for their patience or their communication skills.

Get a second opinion. But prepare yourself for the possibility that the first opinion, while not delivered with tact, may be your best option.

What a great thread for my 500th post .
There have been a couple of posts with great advice .
1 You may just need the partial circumcision
2 Ask to try the cream , nothing tryed nothing gained.
3 Karabulut is right slashers are not known for their bedside manner.
4 DO get a 2nd opinion not only from a urologist but from a plastic surgeon
who has experience in this field.
5 Always remember circumcision should only be used as a last resort.
6 Good luck fella ..|

Too often doctors just want to do what is easiest for them.

Thanks everyone for all of your advice - I contacted my Primary Care Physician and explained my concerns and requested another referral for another Urologist in Houston that I have heard about to get a 2nd opinion. For now I am going to wait until I see the 2nd Urologist and plan on going in with some medical articles about alternative ways to treat phimosis.

A steroid cream is not going to work, it will only make the skin on your penis this and fragile. Why are you so resistant to circumcision? Circumcison make for a much more attractive penis, will allow your glans to be less pointed b/c it is trapped in that tight foreskin.
I had a circq in the urologist's office under local anesthesia and have not regreted it for a moment. I feel my cock has actually grown in length and girth and have much more increased sensitivity. A lot of people will say that the sensitivity will be less but with my experience it increased.
If I had to do it over again I would in a heart beat.
Good Luck!!

...his answer was "The only way to treat this is circumcision" -

...

OH! MY! FUCKING! GOD! :grrr:

Don't walk, RUN to another urologist.

Yes, he is correct: The only way to fix your problem with Phimosis is surgery.
No, it doesn't mean a full circumcision.

Any urologist worth his exhorbitant fee and surgical experience will know how to remove the Phimotic Ring and leave the rest intact. This is what you need to insist upon if you wish to retain your foreskin. It's possible and it works!

Thanks for all of the replies - I am still doing research and talking with my male friends, both gay and straight, to get their opinions. One of my co-workers also recommended his Urologist so I have a third option if the 2nd one doesn't work out.

I am becoming more open to the idea of getting circumcision done if it comes to that but I just want to hear about all of my options. Does anyone know if insurances frown upon getting more than 2 opinions?

Depends on the insurance company. It would be worth a phone call or email to ask though. This is an important health issue - not only physically. Issues with our dicks can also affect us men mentally. So, even if the insurance company says No to a third (or fourth) opinion, if you are not comfortable with the second urologist it would be worth paying for an office visit to the third.

I went to the second urologist today and his recommendation was - you guessed it - circumcision. I explained about alternative ways and while he did discuss them with me he said that he felt it that in my case having the circumcision was going to be the best way to go - now, one thing he did tell me was that because of my foreskin it is likely that even when he cuts off the ring I will still have skin left - i.e. a partial circumcision.

Now while he was a nice guy I did not get a good vibe from him in terms of actually performing the surgery - I went in there just wanting his advice and his opinion. The first urologist actually has an excellent reputation in Houston of performing surgeries quite well.

So I am still going to think about it - because of my job I really can't take any time off during the summer so if I do end up doing the surgery it will likely be sometime in October or November or early 2012. I will also contact my insurance to discuss the possibility of seeing a 3rd urologist but so far two urologist in the past month and one from a few years ago have all said that circumcision is probably my best option and I honestly am beginning to accept that is probably going to be the reality.

so it seems you do have a fibrous ring? depending on how tight it is, removing it might be your only option. you will keep all the skin behind it though. still a lot better than a full circ if you ask me :)

What you gained from the second opinion was not only a confirmation of the first opinion, you also had someone explain to you why that was the recommendation.

In the first opinion, the surgeon laid out the scenarios very bluntly but basically what he said was, "This isn't urgent but it is something that you should take care of".

Sorry to hear about your problem.

I am a medical student and I do agree with the urologists you have seen. Usually if your phimosis is not too bad it can be stretched manually when younger (although I DO NOT suggest that at this point) or use of a steroid cream underneath the foreskin to also try to loosen it up.

If you ever get your foreskin stuck behind the head of your penis for a long period of time where it becomes inflammed and looks like the blood circulation is being cut off, you need to go to the ER immediately.

Most importantly you need to keep your foreskin clean. Is it inflammed at the moment?

The only way to completetly cure phimosis is circumcision unfortunately. Stretching/use of creams have very limited efficacy.

If you want any more detailed info/resources please let me know and I will try to help you out. Just remember that I'm still a medical student and NOT yet a doctor.

Thanks everyone for the replies - I did request a referral for a third urologist that a co-worker recommended. I pretty much think that some sort of circumcision is necessary but I also think it can't hurt to talk to another guy - when it comes to Doctors I have high expectations :)

Secondly - there is no inflammation since I can retract the foreskin behind the head of my penis when I am not erect - so yes when I am showering I pull it back and clean it. The main issue is not pulling it back when I'm erect.
