Never. I've lived in Florida all my life.

No. I haven't ridden a bicycle since I passed my driving test at 17.

I presume you mean, have I ever had my driving license suspended or revoked? No. If you're asking whether I've ever lost physical possession of my license, yes. I once made the mistake of bringing a young man I had just met with me to my hotel room when I visited New Orleans. I took a shower in the morning, and when I got out of the bathroom, he was gone, and so was my wallet.
