How important are first impressions to you?

A smile can definitely drastically change a first impression. I remember once smiling more than usual while ordering food at a fast food place, and the cashier automatically smiled back a (huge) smile that she couldn't wipe off her face. I wasn't even aware that I was smiling that much until I saw her reaction!
 
Very important. But not important to the extent that I'll dismiss the person after the first meeting. It is all about getting to know the person better before gauging if he is bad for you (friendship or romantically).
