I had sex with a guy (a couple of hours ago) and I was the top. I did not use a condom. Big mistake, I don't know why I was so irresponsible.

It was a hook up and I basically don't know him. Should I go get the emergency PEP medication at the hospital now? I am worried. I heard doctors approve it only for those that bottom. Is that correct?

I don't use the daily PREP pill. I asked the guy if he does, and he said yes, but I have no proof that's true or that he takes it daily.

If the issue is causing you this much anxiety, the you should go to a urgent care or an emergency room and discuss the issue with a physician or nurse practitioner. After you have the discussion with them, you can make the decision as to whether you want PEP.

Whether you decide to take PEP or not, you should see your physician for a STD testing. There's a whole lot of other things that you're more likely to get (e.g. syphilis or gonorrhea) that are far more common that HIV and much easier to transmit during anal sex. That visit would also be an opportunity to discuss whether you want to get on PrEP in the future.

Ive been to the baths and I havent seen as much concern as I see in this post. I see guys suck the guys cock after its been in his ass. The 10 times I have been at baths, I cant really think of a time a condom was mentioned. Or maybe Vancouver is just 'dirty'

I went to the hospital and they basically told me that it's my decision. I took the prescription and started taking the pills (less than 24 hours after the sex). I really don't know if topping bareback once is considered such high risk. But I guess better safe than sorry. I am a bit worried the side effects will be really hard for me. We'll see...

What does my question have anything to do with a bathhouse? I had unprotected anal sex as a top. All the sex education I ever got in my life told me that's a big no-no. I should not be worried because people do 10x what I did? Do you know the status of the guys at that bathhouse?

The guidelines say that the "insertive" i.e. top partner in anal is at low risk but in cases like this, it is better to let the patient make the choice for their own peace of mind. There are some side effects that some patients will experience but thankfully, the course of treatment is short, so it's temporary.
