It happens ... it's probably far more common than a MFF threesome happening, since it's easier for couples to find a willing man than a willing woman, even if there's no guy-guy contact.

Personally, I've been in about a dozen MMF situations -- every time, it's been me responding to a couple's inquiry, not the other way around. Sometimes there has been guy-guy contact, sometimes not.
