To what extent are chances of STD/HIV transmission decreased if the top doesn't actually cum inside you? Does it make much of a difference? Just how risky is that?
 
The risk is still quite significant because stds can be transmitted in pre-ejaculate fluid into tiny tears inside the anus. Use condoms!!!
 
For all STDs except HIV the risk is the same cum or no cum.

For HIV the risk is probably slightly lower.

Would you ask "which is riskier, a head on smash at 80 or a head on smash at 100?" - if not, why are you thinking about taking unnecessary risks for sex?
 
Sorry, but even pre-cum contains HIV. No matter how tempting and better it feels, please use a condom always.

STDs are transmitted two ways-
  1. By skin-to-skin contact
  2. By exchange of body fluids (and yes, precum is a body fluid)

ALWAYS USE A CONDOM

The only, and I mean only time you dont have to is when you are in a trusting long lasting monogamous relationship, but even then its a risk.
 
always use a candom.. no matter if you have a long term relation or not
