I've been able to bottom for ten years without a problem. Over the last two years, I feel this really sharp burning pain right when a cock enters me (probably half an inch inwards on one side). All STDs are negative and I have no known hemmroids or fissures. Also no pain when pooping.
I literally take six months off from bottoming each time in the hopes that it's healed, but then the same pain at the same spot happens.

What could it possibly be? Why doesn't it just heal?? And what can a doctor to do check it?

Thanks:)

Not being a trained medical professional i can only make a guess You might have Proctalgia Fugax: https://www.medicalnewstoday.com/articles/317254#causes That has some information for you, without any other symptoms it's unlikely it would be anything else, but not impossible (not trying to worry you, but things manifest differently in people)! Dr. might send you to a proctologist for an anoscopy or an endoscopy sounds scary but they stick a camera up your ass to see whats going on! Call your dr's office tomorrow and speak with your dr. and ask if this sounds like something that can be addressed in their office. It is a high chance this will be a specialist visit as your pcp might not have the necessary equipment, just thought I could save you a dr bill, you'll still have one but at least it aint 2.

The description of a "burning" pain implies that there's more of an issue with the internal anal sphincter. Generally, sharp or immediate pain would be more likely with inflammation or some sort of local irritation. "Burning" pain is more associated with stretching or tension applied to the surface skin or to a muscle.

There are people who can be penetrated and instinctively relax their sphincter so that there is no pain. Most people need some time to relax their sphincter via foreplay or using a toy prior to sex.

Out of curiosity, are you able to use any anal toys and do these cause similar pain? I would be inclined to suggest that you try a dildo, butt plug or prostate stimulator alone, which gives you the opportunity to control the speed and timing of insertion. If you do not feel a burning pain when you use a toy on yourself, it's likely that your experiences with a partner are related to the level of relaxation of your sphincter.

On the other hand, if you are feeling pain when you use a toy on your own, then your next stop is a colorectal specialist to see if there is a hemorrhoid or another cause that needs to be addressed medically.
