I just played this level. I didn't shoot a single civilian, the mission doesn't force you to. The terrorists did all the dirty work.

Actually in that mission you play a U.S. spy working undercover in a terrorist group, who are really using you to start a war with Russia. You get betrayed at the end of the mission.
