It's not as easy as saying on meds means a sane, happy person, and off means a crazed person. There is biological evidence for schizophrenia more than any other mental illness. However, I have heard (not first hand) of people who on meds couldn't cope with the clearer reality of life and became very depressed.
I guess the bottom line is that in taking him on you are taking on some responsibility of assessing whether he needs more help, if he is very mentally distressed, confused, wants to hurt himself, etc. But I guess if he is functioning OK, all is well. And maybe I shouldn't be guessing this stuff at all. You should trust your gut and be concerned--I would in particular ask who his support people are in case you ever needed to call for him.
