Back when I did perinatal rotations, one thing that shocked me was the number of women who came in ready to deliver but who had never seen an OB during their pregnancy. Nada. No prenatal care. No immunizations. No fetal ultrasounds. No prenatal teaching. Many of their babies went to NICU for low birth weight or other complications.

This was before the CHIP program. Before the ACA made more women eligible for Medicaid.

In the early 1990s, there was a measles outbreak in New York City. One thing that we discovered was that many of the children who were infected were unvaccinated because of Reagan-era cuts in public health programs that offered free vaccines and did health promotion events to get the word out that kids needed to be vaccinated. We seem to be reminded every generation that cuts to public health programs can take a decade to reveal the implications.

Why we would ever want to go back to this is a mystery but thanks to the Big Beautiful Bill, that is where we are headed.

Individual vaccines for each virus aren't available in the U.S., and no published scientific evidence shows a benefit to separating the combined vaccine.

Acting CDC Director Jim O’Neill on Monday called on vaccine manufacturers to develop separate shots for measles, mumps and rubella instead of the current vaccine, which combines the three.

O’Neill wrote in a post on X that manufacturers should replace the MMR vaccine with “safe monovalent vaccines,” which only target one virus. His statement referenced a recent comment from President Donald Trump, who advised people last month on Truth Social to “break up the MMR shot into three totally separate shots.”

However, no monovalent vaccines for measles, mumps or rubella are approved in the U.S., and the Centers for Disease Control and Prevention says there is no published scientific evidence that shows a benefit to separating the combined vaccine. It is not clear whether the change O'Neill is calling for is possible or likely to come about.

It's sincere. Marge's district changed. Her new district has a lower median incomes and is more white and more rural.

Marge has her faults but she is in her district a lot and she listens to her constituents, which is why she's liked in spite of her nutty beliefs. She's likely getting an earful from her constituents about higher insurance premiums and from her healthcare organizations about higher uninsured patient burden. It's a long way to Atlanta if you're in labor or you're having a heart attack.

Here's the stupidity of what the Republicans are trying to pull: When asked, "Why do you want to raise insurance premiums on your constituents?". Some Republicans are answering, "Because the premium assistance was supposed to be temporary and was intended to help Americans during COVID". This is a true statement.

Other Republicans are saying, "Because we don't want to pay for the insurance for illegal aliens". This is a White House talking point. And it is a lie.

Republicans, like MTG, are getting real life examples of insurance premiums that will be increasing by hundreds of dollars per month. Because the Republican Party has been trying to lie about the insurance premium assistance, it's caught them in a situation where doing the right thing for their constituents can be turned into a primary challenge commercial that says, "Marjorie Taylor Greene voted with the Democrats to give taxpayer money to illegal aliens to give them free health insurance!".

Frank Luntz, the Republican pollster, has predicted that the Republican base will turn on the Republican Congress and Republican White House in 4Q25. Why? Because Americans spend a lot of money around Thanksgiving and Christmas. That's when they see how expensive Thanksgiving dinner is. That's when they notice that toys and clothes that used to be cheap are now much more expensive. And that's when they start looking for someone to blame.

Add to that the self-destructive stand by the Republicans on extending the subsidies on ACA health insurance plans. Those subsidies allowed a 157% increase in ACA enrollment in "red states". The increase in the blue states was only 36%. During the COVID pandemic, those subsidies doubled the number of people on ACA plans. Why? Because the same "red states" did not take advantage of Medicaid expansion under the ACA. The working poor were not able to get Medicaid but they were able to afford private insurance plans because the Federal government covered the premium cost.


Open enrollment started last week. People are seeing the increased premium costs.

If you really want to piss off people, jack up their insurance premiums by 200%, tell them that they make too much money to qualify for Medicaid and then make them run up big credit card debt because they want Thanksgiving dinner and toys under the Christmas tree.

I wish I could share Luntz's optimism....but I think he is overestimating the ability of Republicons to shift their votes to Dems because for most of them, their racist, transphobic and irrational fears of 'socialism' will still make them check the red box. Back in July, a 27 year old man drove from Las Vegas to NYC and subsequently shot and killed 4 people in a Manhattan office building. The man intended to go to the NFL offices in the building but got off on the wrong floor. The gunman had a a note in his pocket claiming that he had chronic traumatic encephalopathy, (CTE), even though there is no test to diagnose CTE in an alive person; the diagnosis is made only at autopsy.

Press reports at the time were skeptical, since the shooter, Shane Tamura, had never played professional football.

This week, the NYC Medical Examiner confirmed that Tamura had CTE. Tamura had played football as a child into his high school years.

Tragic...at least four guys I went to HS with who played football and were the heavy hitters for the team have committed suicide because of their CTE. I know of at least 2 others
who are basically non-functional at a relatively young age with early onset dementia but just ended up in care.

Could you relax a bit? I knew what they were shooting for back when I learned and read about Project 2025.

It has made me wonder about some of the "dumb jock" stereotypes from junior high and high school. I have some friends with kids in school now whose boys have been treated for depression and brain fog after taking a hit in [American] football games. How much of the behaviors in young men from 30-40 years ago might have been from concussions and early CTE from hits they were taking at practice and during games?

Physicians are becoming more conscious of CTE and concussions. Parents are worried but the boys are still playing football.

I don't see government doing anything about it. And with all the research cuts from DOGE and Vought, it's unlikely that anyone is going to be doing a serious look at the problem any time soon.
